Tears in the foreskin especially at the underside of the penis are very common and usually heal quickly. The problem is when the tears happen over and over again and the scar tissue that forms does not stretch and limits being able to retract your foreskin. This condition is called phimosis. Seeing your doctor for an assessment and possible referral to a urologist would be recommended.
